Survey Promotion Rules
SURVEY PROMOTION OFFICIAL RULES
NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN THIS PROMOTION. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. THIS PROMOTION IS VOID WHERE PROHIBITED BY LAW.
1. SPONSOR. LiveOak Fiber, LLC (the “Sponsor”) will sponsor this promotion (the “Promotion”) as part of its customer survey (the “Survey”) to occur in the first quarter of 2024.
2. ELIGIBILITY. This Promotion is open to Sponsor’s active customers that: (i) completed the Survey and provided their email addresses (“Registrant(s)”); (ii) are legal residents of Georgia or Florida; (iii) are eighteen (18) years or older at the time of this Promotion; and (iv) are current in amounts owed to Sponsor. All winners in this Promotion must meet these eligibility requirements to receive the prizes. Employees (and their immediate family members and members of the same household) of Sponsor and its affiliates, agents, advertising, or promotion agencies are not eligible to participate in this Promotion.
3. ABOUT THIS PROMOTION & PRIZES. This Promotion shall begin on February 19, 2024 and end on February 26, 2024, and shall consist of a drawing (the “Drawing”) with ten (10) winners. The Drawing will take place at the conclusion of the Promotion period.
Ten (10) prizes will be awarded in this Promotion, with only one prize per each Drawing winner. Odds of winning depend on the number of eligible Registrants. Each Drawing winner’s prize will consist of:
• Gift card valued at fifty dollars ($50.00) per winner.
All prizes are non-assignable and non-transferable, and are not redeemable, exchangeable, or substitutable for cash. All taxes and other expenses associated with a prize are each winner’s sole responsibility. All prizes are awarded “AS IS” and WITHOUT SPONSOR WARRANTY OF ANY KIND, express or implied (including, without limitation, any implied warranty of merchantability or fitness for a particular purpose).
4. HOW TO ENTER. Sponsor’s customers may complete a customer survey provided by the Sponsor. Customers that complete the survey and enter their email addresses by the end of the Promotion period will be entered into the Drawing.
5. SELECTION OF WINNER. The Sponsor will notify winners of the Drawing by email.
6. TO CLAIM PRIZE. To claim a prize, each winner must accept the prize within seven (7) days after Sponsor’s sending of the email, by following the directions in the email. If a winner fails to comply with any of the provisions of these Official Rules, such winner will be deemed to have forfeited the prize, in which case Sponsor will award the prize to an alternate winner.
7. RIGHTS GRANTED BY ENTRANTS. By entering the Promotion, each entrant understands and agree that Sponsor, anyone acting on behalf of Sponsor, and Sponsor’s licensees, successors, and assigns, shall have the right, where permitted by law, to print, publish, broadcast, distribute, and use in any media now known or hereafter developed, in perpetuity and throughout the World, without limitation, the entrant’s name, portrait, picture, voice, likeness, image, statements about the Promotion, and biographical information for news, publicity, information, trade, advertising, public relations, and promotional purposes without any further compensation to the entrant.
8. LIMITATION OF LIABILITY. By entering the Promotion and Drawing, each entrant agrees to release and hold harmless the Sponsor and its subsidiaries, affiliates, advertising and promotion agencies, partners, representatives, agents, successors, assigns, employees, officers and directors from any liability, illness, injury, death, loss, litigation, claim or damage that may occur, directly or indirectly, whether caused by negligence or not, from (i) such entrant’s participation in the Promotion or the entrant’s acceptance, possession, use, or misuse of any prize or any portion thereof, (ii) technical failures of any kind, including but not limited to the malfunctioning of any computer, cable, network, hardware or software; (iii) the unavailability or inaccessibility of any transmissions or telephone or Internet service; (iv) unauthorized human intervention in any part of the entry process or the Promotion; (v) electronic or human error which may occur in the administration of the Promotion or the processing of entries.
9. GENERAL. This Promotion and these Official Rules will be governed by and construed in accordance with the laws of the State of Georgia. The Sponsor and the Registrants consent to the exclusive jurisdiction and venue of the federal, state, and local courts located in Glynn County, Georgia for the resolution of any dispute that arises from the promotion or these Official Rules. By participating in this Promotion, each entrant accepts the terms and conditions stated in these Official Rules, agrees to be bound by the decisions of Sponsor, and warrants to be eligible to participate in this Promotion.
