Essential Service Plan Terms
Customer Certification and Legal Disclaimer for ESP Service Eligibility
By checking this box, I certify, under penalty of perjury, that I am currently enrolled in the aforementioned and selected qualifying service program(s) required for eligibility under this Essential Services Program (“ESP”) offering. I affirm that the information I have provided is true, accurate, and complete to the best of my knowledge.
I understand that my participation in this program is contingent upon meeting the applicable eligibility requirements and maintaining my qualifying status. I further acknowledge and agree to the following:
- The service provider may conduct eligibility reverification every twelve (12) months, at which time I will be required to resubmit proof of eligibility and re-attest to my continued qualification for the ESP service offering.
- If I fail to provide valid documentation or attestation during this reverification period, I may be removed from the program and returned to standard retail rates.
- If I am found to have misrepresented my eligibility, enrolled fraudulently, or otherwise violated the program’s requirements, the service provider may, to the extent permitted by applicable law:
- Immediately terminate any discounted or subsidized services;
- Back bill me for the full retail cost of services received while ineligible;
- Pursue all available legal remedies
I agree to notify the service provider immediately if my eligibility status changes or if I become aware of any inaccuracies in the information I have provided. In addition, I agree to enroll in automatic payments (autopay) and to be billed monthly for the ESP service.
